Stage Four: Transition

Key tasks

  1. Identify and secure access to evidence‐based interventions as outlined in Personal Care and Learning. Plan to be delivered onreceiving school site (as part of supporting transition)or in local CAMHS
  2. Peer mentoring‐ link pupil to peers in identified school, if possible a learning peer mentor and a wellbeing peer mentor.
  3. Network and Family conference arranged
  4. Transfer plan for mental health and wellbeing support made with pupil, family, key network professionals and receiving school.
