Key tasks
- Identify and secure access to evidence‐based interventions as outlined in Personal Care and Learning. Plan to be delivered onreceiving school site (as part of supporting transition)or in local CAMHS
- Peer mentoring‐ link pupil to peers in identified school, if possible a learning peer mentor and a wellbeing peer mentor.
- Network and Family conference arranged
- Transfer plan for mental health and wellbeing support made with pupil, family, key network professionals and receiving school.
